• Prescription refill requests require at least 24 hours notice.
• Please provide 24 hour notice of cancellation to avoid a charge.
• If you are more than 15 minutes late for your appointment, you might be asked to reschedule.
• If your child is being seen for a Well Child Check and you have other concerns that are not related to routine, wellness care, those concerns may generate other charges to your insurance. We may also perform other screenings or tests that may not be covered under your preventative benefits.
• Well child care visits are scheduled according to the guidelines of the American Academy of Pediatrics. Please note that this schedule may be different from the one devised by your insurance company.
• Sick visits - Walk in appointments are strongly discouraged. If your child needs to be seen because of an illness, same day appointments during office hours are available. Please call ahead to obtain a time.
• Missing an appointment – We cannot stress enough how important it is that you come to your appointments. We will call and remind you of the appointments 1 day prior to your appointment, as a courtesy. However you are still responsible for keeping your appointment time even if we cannot reach you.   On your first offensive you will be called and mailed a letter.  Multiple missed appointment may result in dismissal from the practice.
• Co pays – As part of our contract with the insurance companies we are legally required by the terms of the contract to collect any co pays from you at the time of service.
• Camp & School Forms – We will complete a health, camp or school form if the patient has had a well check within the last 12 months. All required medical forms must be submitted at least 1 week in advance in order to ensure that all paperwork can be completed and signed by one of our providers.  Once completed you will be called to pick up your paperwork. 
• Payment Policy - Payment for services is expected at the time of your child's visit. This includes co-pays, balance from prior visits and payment in full when we are not contracted with your insurance carrier. We accept checks and credit cards (Visa, MasterCard & Discover). The accompanying parent or other adult is responsible for full payment and providing current insurance information.

We do our best to run on schedule, as we realize that your time is also valuable. There are many ways you can assist us in staying on time:

 - Please be on time for your appointment.  If you are running late please call us and let us know.

 -  If you schedule a visit for one child, please bring that child only. If more than one child needs to be seen, please inform us ahead of time so that enough time is allotted for you.

 - We also realize it's not always possible for parents to bring their child to appointments.  We will gladly still see your child please send the caregiver with a signed note advising us it's ok and how to reach you.
